Regulations of Appointment Service for Patients Discharged and Patients with Chronic Diseases

In order to bring convenience to the patients discharged and patients with chronic diseases, improve appointment service system, based on the patient-centered principle, we set up the appointment service for the patients discharged and patients with chronic diseases.

 

1. Regulations of appointment service for the follow-up examination of patients discharged.

(1) Patients who need follow-up examination in two weeks after discharge can make appointments via several approaches.

 

(2) Patients discharged from our hospital more than two weeks who need follow-up examination can wait for relevant information after making registration with the resident doctors. Within 14 days ahead of the appointment date, staff from the One-stop Service Center will make a further confirmation with you.

 

(3)After the successful appointment, patients will receive relevant message about the appointment on their mobile phones. Patients will be informed of the exact day, the exact expert, and the exact time period for consultation.

 

(4) Patients discharged can make appointments in their corresponding inpatient ward of each department.

 

(5) Line for appointment: 85026079、85027696

 

(6) Please fill in the accurate patients’ ID number of outpatient clinic, name, ID card number and contact phone number.

 

(7) Cautious and accurate job in registration and statistics is required

2. Regulations of appointment service for patients with chronic diseases

 

(1) Regulations of appointment service for the eight kinds of chronic diseases listed by Zhenjiang authorities

 

1) Appointment approaches: mainly by appointments during examination.

 

2) Time for Appointment: normal office hours from Monday to Friday.

 

3) Place for Appointment: reception desk at theOne-stopServiceCenteron the first floor.

 

4) Patients for Appointment: Coronary atherosclerotic heart disease, Chronic pulmonary heart disease, chronic cholecystitis, systemic lupus erythematosus, Cerebral vascular accident recovery and sequelae, Parkinson's disease, or aplastic anemia, Leukaemia, myelodysplastic syndrome and malignant tumors;

 

5) Responsible position is required to ensure the information accuracy of each patient.

 

6) Responsible position is required to ensure the accuracy of record statistics.

 

 

(2) Regulations of Appointment Service for Patients with the membership of “Home of diabetes patients” and “Home of coronary heart disease patients”

 

1) Patients with membership could have consultation fee deducted by three Yuan.

 

2) Regular participation in monthly healthcare activities is expected.

 

3) Health records should be set up.

 

4) Patients with membership could make appointments through various approaches, so as to reduce waiting time.

 

5) Any major changes of the illness state, please go for emergency treatment.
